A register of sports activities with the following information: OIB, Registration number of the Register of Sports Activities, Class of Introduction into the Register of Sports Activities, Date of award of class and registration number in the Register of Sports Activities, Name, Type of Entity, Type of Association, Name of legal entity, Short name of legal entity, Name and abbreviated name of legal person in foreign language, Address of the seat of the legal person (street and house number), city of legal person, Date of the decision on registration in the Register of Associations of the Republic of Croatia or Court Register The City of Legal Entity, Number of decisions on registration in the Register of Associations i.e. Court Register (Class/Order number), Number under which legal persons are registered in the Register of Associations of the Republic of Croatia or Court Register, Date of registration of the legal person in the Register of Associations of the Republic of Croatia, i.e. Court Register, Date of the last held Electoral Assembly, Sport, Sports activity, Persons authorised to represent, Note (History of amendments)
