Local patient and user register
Description
KPR contains information persons who have applied for, receive or have received health and care services from the municipal health and care services, from establishments that provide dental services and from private persons without an operating agreement with the municipality or county that provides health and care services at the municipal level.
When necessary for the purpose of the register, cf. section 1-1 second paragraph, the following types of information may be registered:
a. direct personally-identifying characteristics:
— Personal identification number or other personal identifier
demographic and socio-economic information:
— Date of birth
— Gender
— Municipality of birth/country of birth and time of immigration
— Place of residence/emigration time
— Living and care situation
— Marital status
— Education
— Employment affiliation
— Income
— Date of death
c. administrative information on services:
— Service provider/organisation
— Indication of duration and form of contact with the service
— Whether individual plan, care pay and user-controlled personal assistance have been prepared (BPA)
— Interaction within the service
d. health and care professional information:
— Health status
— Diagnosis
— Functional ability
— Risk factors for disease and malfunction
— Categorisation of/type of service needs
— Type of health care or service
e. information on accidents and injuries:
— Place of injury
— Time of injury
— Other information on damage
f. information on financing of the service
g. information about the data subject has objected to the processing of health data
h. patient and user-reported information
