Description
In Primary Care, On-call Care (ATC) could be defined as extending beyond the regular hours of Team activities, which provides uninterrupted care. This service is aimed at the entire population assigned to the Primary Care Team(s) of the Basic Area(s).
Continuing Care is organized in a similar way throughout the Community, it is offered outside ordinary hours in Continuing Care Points (PAC) of the Basic Health Zones (ZBS) or in Continuing Care Points that group several ZBS (Integrated PAC).
