Description
The dataset contains information about the subject/object of the law and its properties, which is kept in Agenda A4003: National contact point for eHealth.
Basic description: The medical record is a set of information relating to the patient for whom it is kept.
Registered characteristics/data:
4003-5-1: name(s)
4003-5-10: data obtained from the patient's family, personal and occupational history and, where justified, social history data
4003-5-11: Data related to the death of the patient
4003-5-12: other data under this Act or other legislation governing health services or the provision of health care
4003-5-2: surname
4003-5-3: date of birth
4003-5-4: social security number, if assigned
4003-5-5: the number of the insured person of public health insurance, if this number is not the birth number of the patient
4003-5-6: address of the place of permanent residence in the territory of the Czech Republic, in the case of a foreigner, the place of reported residence in the territory of the Czech Republic and, in the case of a person without permanent residence in the territory of the Czech Republic, the address of residence outside the territory of the Czech Republic
4003-5-7: Gender of the patient
4003-5-8: Identification data of the health service provider
4003-5-9: information on the patient's state of health, the course and outcome of the provided health services and other significant circumstances related to the patient's state of health and the procedure for providing health services
