1042-4-2: Identification details of the taxi driver's licence and licence
1042-4-3: Data on the duration and loss of reliability of the taxi driver
1042-4-4: Identification data of the decision imposing a penalty on the taxi driver for an offence as a result of which the taxi driver ceased to be considered reliable pursuant to Section 9(3), as well as the type and amount of the penalty and the legal classification of the offence
