Provides care within the framework of a multi-professional dementia team to people with dementia at home with special service (SoL). Multi-professional team-based work with competencies from healthcare and social services aims to create continuity in care and care for people with dementia and to facilitate assessments that require multiple perspectives to cater for care and care from a holistic perspective.
