Description
outreach activities for people who are acutely homeless; Outreach here refers to a clear function that has the task of conducting outreach and active work to seek out people without housing in need of social services and offer them adequate support. The function also includes supporting the individual to an adequate contact with the care and support system. Acute homeless means that the individual has no place to live and sleep, the person is staying, for example, in shelters, emergency housing, public places and the like.
