Description
For unit level, the value is given as Yes=1 and No=0, while at municipal level it is given as a percentage (%) of units where there are routines for how planning of the individual's health care (care planning) is to be carried out in collaboration with different actors. It is documented in the journal how the individual participated in the care planning. The procedure shall be decided at management level and documented. It must be up-to-date, i.e. followed up or drawn up within the last year.
