For unit level, the value is given as Yes=1 and No=0, while at municipal level it is indicated as a percentage (%) of units where procedures are in place for how the planning of the individual’s health care (care planning) is to be carried out in collaboration with the individual and with the municipal health and health care staff at the region’s health centre and home care staff. The procedure must be decided at management level and documented. It must be up to date, i.e. followed or drawn up within the last year. The answer option “not current” for cases where the region is responsible for all healthcare in ordinary housing (only for municipalities in Stockholm County, Norrtälje exception) is marked as non-response (.).
