For unit level, the value is given as Yes=1 and No=0, while at municipal level it is given as a percentage (%) of units where there are routines for how the planning of the individual's health care (care planning) is to be carried out in collaboration with the individual and with the municipal health care personnel and home care personnel. The procedure shall be decided at management level and documented. The answer option "not relevant" for cases where the region is responsible for all healthcare in ordinary housing (only applies to municipalities in Stockholm County, with the exception of Norrtälje) is marked as non-response (.).
