If applicable, include: Reception visits, home visits and home health care visits. Where applicable, excluded: Day care visits. Visits are the same as the term outpatient visit: outpatient care contact that involves a personal meeting between the patient and the healthcare professional, only outpatient visits that have been documented in the patient’s medical records are reported.
