District assistance: Home and Nursing Care

Description

In 2015 the number of patients followed in scheduled home care (ADP) by the Basic Medicine Services of the Province of Bolzano was 4,459. This value is much higher than in previous years (from 2011 to 2014 on average there were about 2,300 ADP patients). The significant increase is due to the elimination of the lump sum in favour of general practitioners for the care of dependent patients in inpatient centres. After the termination of the provincial collective agreement, general practitioners requested reimbursement for their dependent patients treated in inpatient centres through scheduled home care. Each of the general practitioners involved, equal to 73.0% of those operating in the provincial territory, followed an average of 22 patients. 85.2% of patients followed in ADP were over 75 years of age. The rate of planned home care for the Province is 102 patients per 10,000 inhabitants. Scheduled adult home care was predominantly provided for severe NYHA 3-4 heart failure (21.2%), senile dementia/Alzheimer's disease (21.0%), degenerative bone and joint disease (10.7%) and stroke with paresis (9.3%). In 2015, the number of patients followed in integrated home care (ADI) by primary care services was 760 (731 patients in 2013 and 725 in 2014). The rate of integrated home care for the Province is equal to 17 patients per 10,000 inhabitants. 54.6% of general practitioners from the Autonomous Province of Bolzano were involved in integrated home care programmes in 2015; On average, 2.1 interventions per month were carried out on each patient, with the contribution of different professional figures. 68.4% of the patients followed in ADI were over 75 years of age. Overall in 2015, 39.3% of patients followed at home in an integrated form were affected by tumors, 11.6% were affected by severe heart failure NYHA 3-4, 9.1% by stroke with paresis and 8.5% by senile dementia/Alzheimer's disease. The direct-to-user nursing services recorded in 2015 throughout the provincial territory were 1,433,784. The most frequent activity is the collection of venous blood with 220,358 services, it follows the education of the person and / or family for the purposes of prevention, hygienic-sanitary empowerment and adherence to the intervention program (149,026 services). In 2015, nursing staff also played a very important role in the activity of measuring vital parameters and/or assessing the status of one or more organ-systems with 141,239 performances and in the preparation and/or administration of drugs (141,130 performances).
