Allergen Chip Challenge

**Doi 10.60597/j5fe-g420** The steady increase in allergies requires improved patient diagnosis, which is based on clinical history, skin testing (CT) and detection of specific IgE antibodies (IgEs) in the blood. The aim is to identify or exclude potentially implicated allergens. We currently have multiplex IgEs tests based on chips that allow simultaneous detection of hundreds of allergenic specificities with only 100 μL of serum. For example, Allergy Explorer ALEX2 (Macro-Array Diagnostics, Vienna, Austria) uses 117 allergenic extracts and 178 protein components. This benefit of detecting IgEs at many allergens in one test is offset by difficulties related to interpretation. The Allergen Chip Challenge data challenge was a first attempt to solve this interpretation difficulty. The objective of the challenge was to develop an artificial intelligence (AI) interpretation algorithm capable of predicting the presence and severity of an allergic disease based on its chip-derived IgE profile. It was a collaborative project led by the Société Française d'Allergologie (SFA) in partnership with the Health Data Hub. This project financed by the Public Investment Bank (PIB) was carried out between December 2021 and November 2023. To create this data challenge, the SFA coordinated **the collection of data from 4271 patients with their history and IgEs data with 12 French allergology laboratories from the AllergoBioNet network and their clinical counterparts** (Bordeaux, Lille, Lyon, Marseille, Toulouse, Trousseau, Bichat, Caen, Reims, Clermont, Dijon and Nîmes). The CCA database was retrospective and included data from 2012 to 2023. Clinical data were collected in patient servers and IgE values in laboratories performing the chips. A total of 292 challengers from around the world submitted 3135 algorithms between May and July 2023. The best algorithm had a high F allergic patient discrimination score (0.786) and high-quality coding to predict respiratory allergies. However, it had the limit of overestimating the severity of allergies and not correctly identifying food allergies and hymenopteran venoms [https://www.health-data-hub.fr/actualites/allergen-chip-challenge-decouvrez-les-results-de-la-competition]. These were presented at national and international congresses (AAAAI 2023 and 2024, EAACI 2023 and 2024, CFA 2023 and 2024) and at an awards ceremony held on 17 November 2023 in Paris (https://www.youtube.com/watch?v=QgyGJVJbh0). Today, we are working on the characteristics and improvement of this winning algorithm. **This database is intended for healthcare professionals and researchers wishing to exploit allergy data combining both clinical and biological data. It can also be linked to other data, in particular environmental data, due to the link between allergic diseases and the conditions in which patients live (exposome).** A first wave of allergic diseases occurred about fifty years ago with a sharp increase in the prevalence of asthma. We are now experiencing a second wave of allergic diseases, which has resulted in a sharp increase in the number of people suffering from food allergies. This phenomenon is linked in particular to climate change (heat wave, pollution, longer and stronger pollination, pollen-food cross-reactivity, increased concrete surface in cities, etc.) and new lifestyle habits (new foods and allergens, use of detergents, excessive hygiene, use of antibiotics, etc.) (Haahtela et al., 2021). Today, more than 150 million Europeans (20%) suffer from chronic allergic diseases such as asthma, rhinitis and food allergies, with an increasing number of serious and fatal clinical forms (anaphylactic shock) (WHO, 2020). The World Health Organization (WHO) estimates that half of the world's population will be affected by at least one allergic disease by 2050 (Pawankar et al, 2013) and ranks respiratory allergies as the 4th most common chronic disease after cardiovascular disease, cancer and HIV. This explosion of allergies has raised awareness: on 9 March 2023, a day was organised on the subject by the French Ministry of Health, entitled Tomorrow All All Allergic? Taking action to prevent (https://sfa.lesallergies.fr/wp-content/uploads/DP-colloque-FFal.pdf) and on 14 December 2023 at ARS Nouvelle Aquitaine. Nevertheless, allergies are often overlooked: Up to 90% of patients are untreated or inadequately treated while a simple rhinitis actually significantly affects various aspects of life, such as sleep, sex life and physical fitness. This has major socio-economic consequences such as absence from work and particularly low productivity. For the European Union, the avoidable indirect costs for patients who are insufficiently treated for an allergy are between 55 and 151 billion euros per year, or 2405 euros per year per untreated patient (Zuberbier et al, 2014).
