Description
Legal basis for keeping/publishing: Article 5(1) of Regulation No 5 of 6 July 2011 on the conditions and procedures for obtaining a permit for the storage and sale of medicinal products by doctors and dentists and their supply of medicinal products and Decree No 214 of 25 March 2016.
Contains data on the authorisations issued by RZI-Smolyan for the storage and sale of medicinal products by doctors and dentists in accordance with Section II of Regulation No 5, including: the number and date of the authorisation issued; details of the registration number, lot, volume and page under which the health institution is entered in the register of the RHI; name and address of the outpatient hospital (activity address); in the event of termination of the authorisation, the date of termination of the authorisation and the reason therefor; in the event of a change of circumstances, comments on the listed circumstances.
The data shall be updated every time the register is updated.
