Description

Diabetes mellitus, with its complications, is one of the major health problems of economically evolved countries and its prevalence is constantly growing, also due to the increase in obesity and sedentary lifestyle. The prevalence in the literature is estimated at around 5.5% of the general population (ISTAT, 2014), but there is an additional 3% of subjects with undiagnosed diabetes. There are 2 main forms of diabetes mellitus: Type I diabetes (8% of cases) is a childhood-to-youth form that requires insulin treatment. Type II diabetes (more than 90% of cases) is characteristic of adulthood - senile and is often controllable with diet, exercise and / or oral hypoglycemics. In 2015, 935 type I diabetic patients and 20,799 type II diabetic patients (expressed in years lived) were treated in the Province of Bolzano, with a prevalence of 0.2% and 4.1% respectively in the total assisted population. The prevalence of type II diabetes patients progressively increases with age: 12.4% between 65 and 74 years and 16.0% after 75 years. The average age of the type I diabetic population is 39.1 years, type II 69.4 years. The total prevalence of the disease is not homogeneous within the provincial territory. In large urban centres, there are standardised prevalence rates above the provincial average: This is due to a greater use of the Provincial Health System by diabetic patients living in urban centres. The districts with the lowest prevalences are the Valle Aurina, Val Badia, Val d'Ega, Val Gardena and Val Sarentino. At the provincial level three quarters of the type II diabetic population also suffers from cardiovascular disease, more than two thirds is also hypertensive and one third also has dyslipidemia. In the Province of Bolzano, more than 75% of all diabetics carry out at least one annual monitoring of creatinine and glycated hemoglobin. Regarding pharmacological therapy, during 2015 67.2% of type II diabetic patients took an oral antidiabetic, 49.5% a lipid-lowering agent, 32.0% an antiplatelet and 16.9% were treated with insulin; In general, 84.1% took at least one of these drugs during the year. It should be considered that the pharmacological treatment of diabetes involves in most cases the combination of several drugs. The rate of hospitalization for diabetes in the Province of Bolzano is 59.6 admissions per 100,000 residents (excluding admissions made at Austrian facilities and in the rest of Italy). If we consider only the population over the age of 18, the rate is reduced to 68.9 admissions per 100,000 residents. Considering patients over 18 years of age, the rate of hospitalization for diabetes with long-term complications (kidney, eye, neurological, peripheral circulatory or other complications) is 27.1 admissions per 100,000 residents.
