Name of the applicant of products, country, location 3. Product name, country, location 4. Objects of application 5. Number of the certificate of state registration of disinfectant and date of issue (date of introduction of disinfection agent in the register) 6. Validity of the certificate (up to) 7. Institution, institution of the state sanitary-epidemiological service (expert commission), which issued the conclusion of the state sanitary and epidemiological examination
